• Child’s Birth Certificate
• Immunization Records
• Proof of Residency (dated within 30 days)

Required: Log in to FinalForms and update your student’s info. You’ll also need to upload a current proof of residency.
🏀 ATHLETES:
All athletic registrations will now be required to be completed through FinalForms. Every student-athlete must register to participate in school sports.
